How to Switch Your Home Care Provider
Switching to Trilogy Care is easy! Just follow these steps:
Notify Your Current Provider: Let them know you want to switch and set a termination date (1-2 weeks notice is recommended).
Reactivate Your Referral Code: Call My Aged Care at 1800 200 422 or do it online.
Contact Your Coordinator: Share your referral code with the team at Mayflower Supports and start the onboarding process.
